ui: settings forms cleaner — stacked labels + full-width inputs

Bare <label>s inside a .set-form now inherit the clean .set-field style
(label text above, input 100% wide, consistent padding/focus). Fixes the
messy SMTP card (uneven input widths) and the Google form at the same
time. set-form max-width 460px for readability; .set-actions as button row.
