Phase 4: Fan login (Google) behind premium gate

fanLoginReady() = googleConfigured() && premiumUnlocked(). The "Sign in
with Google" button, /auth/google-start, and the callback are now gated on
this (premium off = freely available, demos unchanged; on = Patreon
required).
