feat: embed pause applies to every embed (not just .folio-embed)

Broaden the blur handler that pauses the site player: now triggers on focus
to any iframe instead of only iframes inside .folio-embed. On the site,
iframes are by definition embeds (YouTube/Spotify/SoundCloud/Vimeo/Apple Music/
Bandcamp), so this covers all platforms without gaps. Cache-buster v=12 -> v=13.
