ui(nav): scroll to top when navigating to a different post

Links with their own hx-get (post-nav Newer/Older, post cards) do not go
through the link boost and therefore did not scroll back to the top. A
global htmx:afterSwap handler now scrolls to the top whenever #pcms-main
swaps (in-page/OOB swaps excluded).
